Initial Appointment 

• First appointment must include an additional half hour (1/2 hr) to discuss medical history and complete / review intake form: $30.00         

Caregiver Training 

• Combined with a scheduled appointment / ask for details: $30.00       

We are happy to work out a discount plan with you based on pre-scheduled/pre-paid appointments:     

• Book 5 or more pre-paid / pre-scheduled appointments = $20.00 savings off each appointment                                                                      -
